Here at Hospice Whanganui, we believe in supporting our patients to live the best life possible with the time they have left. Through the provision of excellent palliative care, we support our patients and their whānau in achieving their goals and wishes as they deal with life-limiting illness.We have an exciting new opportunity available for someone to join our amazing team as Whānau Support Navigator/Social Worker. This person will guide and support our patients and their whānau through their end-of-life journey, using a whānau-ora approach that is both whānau centred and whānau led.Sitting within the Whānau Support Team, the purpose of this role is to help identify the psycho-social, emotional and bereavement needs of our patients and their whānau, and to ensure that they get the right supports at the right time. As a Navigator/Social Worker you will work directly with patients and their whānau to support them, alongside our multidisciplinary team; but you will also work closely with GP teams, aged care facilities, Māori health providers, the hospital team, government agencies, home-based care providers, social work services, counsellors, and other community services.We believe that all people, regardless of who they are or where they live, should be able to access excellent palliative care and support. For this reason, most of our work is done in the community, in the homes of our patients and their whānau and in the places that they choose to be. This role will give you an opportunity to connect in a meaningful way with patients and their whānau, to help them do things their way, and in the environment that is most meaningful to them.The core values of Hospice Whanganui are:
