If you are passionate about supporting individuals and whānau on their hauora journey, and enhancing their capacity to take care of their own wellbeing, then this could just be the role for you.As a key member of a multi-disciplinary primary care team, you will be contributing to the collective knowledge and support for the wellbeing of our community.You can be based from either our New Plymouth or Hawera office (flexible depending on where you live) and be joining a great work environment - collaborative, flexible and future-focused. Relevant social work experience, particularly in a community setting.
A passion for improving patient health through holistic best practice.
Demonstrated knowledge and experience of Māori models of health and wellbeing.
A keen ability to develop and maintain key relationships within a multi-disciplinary team, external agencies and supports.
Registration with the Social Workers Registration Board New Zealand.
