Please do not apply if you are not employed by Te Whatu Ora /Health New Zealand StaffAbout the RoleThis role is a secondment to cover parental leave for up to 11 months.The community nurse provides a holistic case management model for children and young people in the community setting with a strong focus on empowerment supporting whānau to manage their child - s health condition. This is not limited to the home and may take place in the outpatient setting, primary healthcare practices or schools. The role encompasses a range of nursing assessments, interventions and evaluations for acute and long term conditions. The role includes inpatient support and consultation facilitating complex patient discharges.The role works alongside other agencies and interdisciplinary teams to manage children and young people with high health needs and health disparities. Case management of eczema and continence patients and wound care management. The team work collaboratively to manage changing workloads across both sites.About the Team/Service/ LocationThe Community team is a small team of 7 Nurses across 2 bases providing support and care for short term patient referrals and case management of complex long term patients and comprehensive assessments of health, social, developmental and functional needs of the child/young person. The team work in partnership with children and whānau experiencing significant health disparities and limited access to healthcare services. This role interfaces closely with Paediatricians and the wider Interdisciplinary team to provide the best outcomes for children and young people.About youThis role is a senior nurse role and requires the successful applicant to demonstrate:
  • Extensive experience in Paediatrics across the age range from birth to 16 years - Working towards Expert on PDRP
  • Knowledge of health disparities working with Māori and Pacific families
  • Prior experience in a community setting working an advantage and leadership experience
  • Working towards Post graduate study.
  • Prior experience educating and teaching- children, whānau and interdisciplinary health professionals.
