- On-site development of service plans, including household management, personal cares, manual handling, pressure areas

Key Requirements:
- A Registered Nurse with current APC and New Zealand experience
- Preferably up to five years New Zealand post-graduate experience including one year in the community, primary health, rehabilitation, disability or aged care sector
- Wound and catheter care experience
- Great communication and interpersonal skills
- Proven skills in health assessment, planning and goal setting
- A full Driver's License
- Demonstrated ability to work collaboratively with other professionals and the aged care/disability sector to maximise health outcomes and goal achievements for client's and their families.
- Confidence in managing IT applications and the ability to report regularly on services provided and outcomes achieved.
