- Develop strength-focused care plans that support the holistic well-being of rangatahi.
- Work collaboratively with whānau and external stakeholders to ensure effective and integrated service delivery.
- Identify risk factors and develop risk management plans, liaising with services such as CAMHS, Oranga Tamariki, and Police where needed.
- Maintain accurate, high-quality documentation and contribute to service development and improvement.
- A full registration and current Annual Practicing Certificate under the HPCA Act 2003 (e.g., Registered Nurse, Social Worker, Occupational Therapist, AOD Clinician).
- Experience working with rangatahi experiencing mental health and addiction challenges.
- Knowledge of evidence-based interventions such as talking therapies.
- Strong communication and relationship-building skills.
- A full, clean NZ driver's license.
- 3+ years of experience as a registered health professional.
- Experience working with diverse cultures, particularly Māori and Pasifika communities.
- Knowledge of community resources and support networks.
- Competitive salary ($84,512.11 - $102,000, depending on experience).
