Hours include: Monday to Friday 8:30 am to 5:00pm.The Clinical Nurse Specialist works across the Nelson Marlborough services supporting tāngata whai ora with their holistic health needs. The role exists because tāngata whai ora frequently have co-existing physical health concerns, and Pathways seeks to ensure their total wellbeing. The CNS liaises with the clinical team including GPs and works with Team Coaches and kaimahi to provide the nursing service to achieve wellbeing.In this role, you will provide expert nursing leadership and deliver culturally safe, evidence-based care to tāngata whai ora and taiohi. You will work collaboratively with a multidisciplinary team-including GPs, Team Coaches, and kaimahi-to ensure integrated and responsive support. You'll assess and manage complex cases, contribute to case reviews, and offer clinical input across all Nelson Pathways and Real services. You will also mentor and guide team members to strengthen clinical capability and improve outcomes for the people we support.Responsibilities include:
- Using advanced health assessment and clinical decision-making to manage care for whai ora and taiohi with complex or co-existing needs.
- Providing clinical leadership through both direct and indirect engagement with tāngata whai ora, taiohi, and the wider addictions team.
- Evaluating the effectiveness of interventions, revising care plans as needed, and initiating timely referrals to appropriate services or agencies.
- Ensuring all interactions are respectful, culturally appropriate, and sensitive to individual needs.
- Applying trauma-informed care strategies in day-to-day practice.
- NZ Registered Nurse with physical and mental health experience
- Hold a current Annual Practicing Certificate
- Recent extensive (proven) clinical experience in Mental Health and Addictions
- Demonstrated ability to articulate evidence-based expertise to a multidisciplinary team.
