You will be reporting to the Team Coach and joining our Pathways Auckland Kelvin Road service, based in Papakura. You will be working in a team of Support Workers.You will be supporting Tangata Whai ora with all health appointments, flat cleaning, shopping, cooking, personal care and laundry, activities and engaging with Whanau when they are on site.This is a permanent, full-time role working 40 hours per week. Your roster will be;
Sunday & Monday, 7am - 3:30pm
Tuesday - Thursday, 1:30pm - 10pmResponsibilities include:
- Advocate and support whaiora to be well and thrive.
- Connect them with resources that support them in living well and assist them in building resilience
- Help people connect with their communities, building the relationships that matter most
- Foster positive change for a more positive life
- Provide medication support for our whaiora
- Support whaiora with their appointments
- Understanding and commitment to principles of recovery
- Current full NZ driver's licence
- Current first aid certificate
- Applicants must hold the New Zealand Certificate in Health and Wellbeing (NZCHW) or a recognised equivalent
- Support Worker experience or similar
- Sound knowledge and understanding of the primary care sector
- Knowledge of, and demonstrable commitment to, the principles of the Te Tiriti o Waitangi
