- Facilitating efficient and effective community-based support solutions for our clients
- Completing client related assessments
- Developing and maintaining person centred individual support plans
- Managing homecare support packages for clients with disability and/or serious injuries
- Liaising with external support providers and funders including ACC, DHB and Allied Health services etc.
- Ensuring our support workers have the right skills, training and support to provide an exceptional level of support to our clients.
- Delegation and direction of support tasks and medication/interventions to support workers
- Problem solving to develop innovative and sustainable support solutions
- A NZ registered nurse with a current practicing certificate
- Community nursing with experience in primary health, complex disability and rehabilitation preferred.
- Full drivers' licence and a reliable vehicle
- Able to perform autonomously and as part of a team
- Excellent communication and documentation skills
