In every aspect of our work, we are committed to helping whānau be the best they can be.He kōrero mō te tūranga | The RoleCommunity Support Worker's with Access and Choice aim to connect directly with whaiora who are referred via their GP practice. This involves working alongside whaiora at their GP practice to develop and complete an identified goal for up to three months to improve their health and wellbeing. This role is walking alongside whanau to give them the tools, confidence, knowledge, and guidance of their community. For example, navigating social services, developing personal plans, gaining employment, budgeting, and housing.You will also be an integral part of the general practice team and working in collaboration with other community partners, as well as building new relationships through networking.This is a 32 hour a week position working Monday - Friday.He kōrero mōu | About YouWe're looking for a positive, flexible, and compassionate individual to join our team! Strong relationship-building skills, combined with the ability to maintain professional boundaries, are key to success in this role.   • A Level 4 (or higher) NZ Certificate in Health and Wellbeing - Mental Health Support, or equivalent.
  • Experience in recovery-focused mental health support.
  • Skilled in managing challenging behaviours with a calm and patient approach.
  • Strong personal and professional boundaries.
  • Committed to recovery-focused values.
  • Full NZ driver's licence.
  • Must have the right to work in New Zealand.
